Initial Meeting Form for Counseling

Thank you for your interest in pursuing counseling. Please take a moment to fill out the following form (access through yellow button below) so that we can understand how we might be able to best serve you. After you complete the form, an RCM ministry leader will be in contact with you within 48 hours to follow up and discuss setting up an initial meeting with one of our biblical counselors.

We offer counseling from professionally-trained biblical counselors. Our counselors are professionally-trained unlicensed counselors (masters and doctoral level training). If your church is a partner with RCM, our fee of $60/hour is paid by the church, and counselees are asked to consider a donation of $30/hour to their church, or whatever amount they are able, to help their church with the cost. The $60/hour cost is a reduced rate made possible by RCM’s group of generous financial partners. If you are meeting with a counselor and not part of a partner church, you will cover the $60/hour cost, and we have a scholarship form you can share with your church if you need help with the cost.

The information you provide for us in the form below will only be shared with your pastor who oversees this ministry (for those who are requesting counseling from a RCM partner church), and it will remain confidential.
